REGISTER - JGFA PROGRAM

• Every boy and girl aged 5-16 welcomed in the JGFA program
• Program consists of four, 10-week terms per year

Parent / Guardian Agreement: By submitting this form you agree to the following: I agree to let my child/team participate in the JGFA program. I understand that there are certain risks of injury in the participation of this sport and I am willing to assume these risks. I confirm that my child/team is capable of participating in soccer and that my child/team is in good physical condition. In addition to giving full consent to my child’s/teams participation, I waive release and hold harmless JGFA, and its members, coaches, and representatives, for any injury that may be suffered by my child/team members. I grant permission for my child/team to receive emergency medical treatment. I grant JGFA permission to use photographic images of my child in its promotional activities.
