Parent / Guardian Agreement: By submitting this form you agree to the following: I agree to participate in the JGFA Champions League 5 v 5s tournament. I understand that there are certain risks of injury in the participation of this sport and I am willing to assume these risks. I confirm that I am capable of participating in soccer and that I am in good physical condition. In addition to giving full consent to my participation, I waive release and hold harmless JGFA, and its members, coaches, and representatives, for any injury that may be suffered by myself. I grant permission for myself to receive emergency medical treatment. I grant JGFA permission to use photographic images of myself in its promotional activities.
